NC teen has rare brain infection likely caused by swimming. Where did exposure happen? Are you safe?

A teenager in central North Carolina is critically ill after contracting a rare and dangerous brain infection caused by a microscopic amoeba called Naegleria fowleri. The amoeba is naturally found in warm freshwater environments such as lakes, ponds, and rivers. Health officials announced the case on Thursday, but as of Friday had not publicly disclosed where the teen may have been exposed or whether that water source remains in use. The North Carolina Department of Health and Human Services said it is coordinating with the CDC and local health departments to investigate.
Naegleria fowleri causes a condition known as primary amebic meningoencephalitis, or PAM, when contaminated water enters the body through the nose and travels to the brain. The infection progresses rapidly and is nearly always fatal. Crucially, the amoeba cannot be swallowed to cause infection — the risk is specifically nasal exposure. Because the organism occurs naturally in the environment, it cannot be completely eliminated from freshwater sources, making prevention and public awareness especially critical.
Current methods of testing freshwater for Naegleria fowleri are complex, expensive, and require advanced laboratory equipment, meaning routine surveillance is not yet feasible. Researcher Joe Brown at UNC Gillings School of Global Public Health is working to develop a faster, cheaper alternative — ideally a roughly $20 test that delivers results within a few hours. Such a test could help public health agencies pinpoint hotspots and track spikes in concentration, giving communities much earlier warning of potential risk.
The limitations of existing testing methods were painfully illustrated by the 2021 death of six-year-old Aven Moffat, who contracted PAM after swimming in freshwater on private property. His mother, Debra Moffat, said the state advised her family to simply assume the amoeba could be present in any freshwater rather than testing the specific site. She also acknowledged that a single test may not be definitive, since water conditions fluctuate with temperature, rainfall, and time, meaning risk levels can change significantly from one day to the next.
Both Moffat and researcher Brown emphasize that public education is a vital tool for reducing infections, particularly because many families are entirely unaware of the risk. Moffat described herself and her husband as well-educated people who had never heard of the amoeba before their son's death. Brown noted that informed individuals can meaningfully reduce their exposure by changing how they interact with warm freshwater — for example, by using nose clips or avoiding activities that force water into the nasal passages in potentially contaminated environments.
